The REPLICCAR III trial is testing whether a smartphone-enabled ERAS platform can reduce length of stay after CABG across São Paulo's public cardiac surgery centres. Using a stepped-wedge cluster RCT design, the 480-patient study offers a potential blueprint for digitally-enabled perioperative care in resource-constrained health systems worldwide.
